Unstable pelvic ring injuries continue to be challenging manage. Obese patients have a higher risk of complications and such management entails additional difficulties for emergency services the medical-surgical team. A 41-year-old male with body mass index 44 fell from water slide suffered an anteroposterior compression impact injury. He presented Tile C1.3 fracture, Denis III sacral Roy-Camille Type 4 spinopelvic dissociation. We performed posterior triangular fixation anterior double plating, patient gained full recovery was able return work. This case report focuses on some key tools techniques consider when managing complex fractures in obese patients, requiring specialized equipment or adjustments conventional treatments obtain good outcome. used accessory portals through fat pad adequately place plate screws.
